Workers' Compensation (August 2024)
The notice now states that an employee may file an Employee’s Claim (Form 110) if an employer fails to report an injury.
The section on medical treatment has been rewritten in its entirety. The statement that “The employee may select his or her own physician” was changed to “Injured workers may select their own medical provider.” The statement that “reasonable and necessary costs of the services provided by the treating physician will be paid by the insurer if the treatment is connected to the work-related injury” was changed to “Medical treatment costs that are reasonable, necessary, and related to the work injury will be paid by the above-named insurer.”
Some text has been moved or excised.
Some blanks on the notice have been removed while new blanks have been added. (For example, there is now a blank for the employer or insurance carrier to fill in the phone number of the medical facility.)
A QR code has been added, which takes the reader to a mass.gov webpage on workers’ compensation policy.
